A continuity of care model, explained

Complete Midwifery Care is full continuity of care with one known midwife throughout pregnancy, birth, and the early weeks after your baby arrives.

This model of care is for women who are planning a homebirth, or who feel hospital is the safest place for them and want their birth approached with respect for physiology, informed choice, and steady support.

I work with a limited caseload each year so that care can remain unhurried, relational, and deeply attentive. This is a premium, one to one model built around trust, presence, and continuity.

If you are seeking care that honours both safety and sovereignty, this page will help you understand whether this offering is the right fit.

This model of care is often a good fit if you:

  • Are planning a homebirth, or a physiological hospital birth

  • Want your care grounded in evidence, not routine intervention

  • Value shared decision making and informed consent

  • Want advocacy and steadiness if plans change

  • Are looking for care that spans pregnancy, birth, and the postnatal period

This model is designed around continuity and an on-call relationship.

If you are looking for one off or targeted support, or prefer an exclusively obstetric led approach, another model of care may suit you better.

What is included

My promise is transparency and comprehensiveness. Everything required for safe, personalised, continuous midwifery care is included.

All antenatal appointments in clinic, at home, or via Telehealth

Continuous labour and birth care at home or in hospital

A trusted second midwife present at homebirths, in line with Australian guidelines

Postnatal visits for up to six weeks, in clinic or at home

24 hour on call support from 37 weeks until your baby is born

Breastfeeding and newborn care support

Advocacy and birth planning, including VBAC preparation or previous birth trauma

All standard clinical checks, referrals, prescriptions, and documentation

This is care that holds the whole journey, not just the appointment schedule.

Birth settings and continuity of care

With private midwifery care, you have flexibility in where and how your baby is born, held within a continuous relationship.

Homebirth

You are supported in your own space by myself and a trusted second midwife. Care is calm, attentive, and grounded in safety, experience, and respect for physiology.

Hospital birth

I attend as your known midwife and care provider. I hold visiting access at the Royal Hospital for Women in Randwick. Care focuses on continuity, advocacy, and protecting the conditions that support normal birth within the hospital setting.

Planned flexibility

Some women plan a homebirth and later decide hospital feels right. Others plan hospital birth and want to labour at home for as long as possible. If transfer is needed, I remain with you, providing continuity and advocacy throughout.

Your care adapts to your needs and your choices, not the other way around

Why continuity of care matters

Research consistently shows that women cared for by a known midwife are more likely to experience normal birth, establish breastfeeding, and feel satisfied with their care.

But beyond outcomes, continuity changes how birth feels.

You do not need to repeat your story.

You do not need to wonder who will walk into the room.

You are met by someone who already knows you.

Continuity allows trust to build over time. It creates space for honest conversations, thoughtful planning, and calm decision making when things are uncertain. It means you are not navigating pregnancy or birth alone, even if the path changes.

If care becomes complex

Pregnancy and birth do not always follow a straight line. Sometimes additional medical input is needed.

If this happens, I remain your lead midwife. I continue to provide continuity, advocacy, and emotional presence, while integrating specialist or obstetric care where appropriate.

You will not be left to navigate systems on your own. Whether your journey is straightforward or complex, you are held within a relationship that does not disappear when things become challenging.

What is not included

To keep things clear, the following sit outside my midwifery fee:

  • Blood tests, ultrasounds, and specialist consultations, usually Medicare rebated

  • Medications dispensed by pharmacies

  • Optional birth supplies such as pool liners or hoses

  • Travel outside my service area, where a small additional fee may apply

  • Additional medical or complementary therapies if required

These costs are discussed openly and early.

About Jen

I am an endorsed midwife with over seventeen years of experience supporting women through pregnancy, birth, and early motherhood.

My work is grounded in evidence informed practice, respect for physiology, and the understanding that birth is both a clinical event and a profound human threshold. I listen carefully, advocate clearly, and pay close attention to the details that help women feel safe.

Care is offered from my clinic in Paddington, supporting families across Sydney’s Eastern Suburbs. I also offer homebirth care in the Lower Blue Mountains region near Springwood.

Alongside midwifery care, I also offer an optional pathway for women who wish to engage pregnancy and birth as a rite of passage.

This is not clinical care. It is deeper, ritual based work that uses reflection, symbolic practices, and integration to support the inner transitions of pregnancy, birth, and early motherhood.

Some women find this work deeply meaningful. Others do not feel the need for it at all.
