This policy explains how I collect, use, store and protect your personal and health information, in line with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the Health Records and Information Privacy Act 2002 (NSW).

As a registered health practitioner, I'm covered by the Privacy Act regardless of business size. The small business exemption that applies to many private businesses doesn't apply to health service providers.

What information I collect

Depending on the service you engage, this may include:

  • Name, date of birth, contact details and address

  • Medicare number, private health fund details, and other billing information

  • Emergency contact and GP details

  • Health information you provide through intake forms, in consultations, or in communication with me, including obstetric, medical, and psychosocial history

  • Clinical notes, examination findings, pathology and scan results, and records of care provided

  • Payment and billing records

  • Website usage information collected through standard website analytics

You're never required to answer a question you're not comfortable answering. Some fields on my intake forms are optional for this reason. Declining to answer a question may affect my ability to provide certain aspects of care, and where that's the case, I'll tell you.

Why I collect this information

To provide clinical care, manage your bookings, process payments and Medicare claims, respond to your enquiries, communicate with you about your care, and meet my legal and professional obligations as a registered midwife, including record-keeping requirements set by the Nursing and Midwifery Board of Australia.

How information is stored

Clinical and booking records are held in Splose, my practice management system. Splose stores Australian client data within Australia, encrypts information in transit and at rest, and maintains redundant, encrypted backups. My website is managed through Squarespace. Payments are processed through Stripe. If you subscribe to my email list, that's managed through Flodesk.

Records created before July 2026 are held in a closed archive from my previous clinical system, retained securely under the same privacy protections described in this policy.

AI-assisted documentation

Some of my documentation is assisted by AI tools built into Splose. This is explained fully in my separate AI Use Policy. You can decline AI-assisted documentation for any appointment without it affecting your care.

How long I keep information

Health records are retained for a minimum of seven years from your last contact with me, or until you turn twenty-five if you were a minor when the record was created, in line with NSW law. Because antenatal and birth records also concern your child, I retain maternity records for twenty-five years from the date of birth as standard practice.

Disclosure of information

Your information is not sold or used for anyone else's marketing. I only disclose it:

  • To other health practitioners involved in your care, with your consent, or where necessary for your immediate care

  • Where required by law, including mandatory reporting obligations that apply to registered health practitioners

  • To third-party platforms that support the operation of my practice, as listed above

  • With your specific consent, for any other purpose

As a registered health practitioner, certain legal and professional obligations may require me to disclose information without your consent in limited circumstances, for example, where there's a serious risk to someone's safety, or where the law requires a report to be made. These obligations exist regardless of what platform or process is used to record information, and this policy doesn't override them.

Your rights

You can ask to see your health record, or ask for a correction if something in it is inaccurate. Contact hello@jenniferhazi.com.au to make either request.

Website usage

My website uses standard analytics tools to understand how it's used. This doesn't identify you personally unless you've submitted a form or made contact.
